Part or Full time Performer dentist required for 3-5 days in a busy, mixed, well established practice near Allerthorpe, East Riding of Yorkshire. 3000-8000 UDAs at £13 per UDA plus 50% private potential approx 1-3k gross a month, 50% labs. Existing, well maintained list plus some new patients. Fully computerised, xrays, rotary endo, and full support team. Hours 8.30am-5pm. 25 minutes to Beverley, 1 hour to York. Available now, negotiable start date.

- Full NHS Performer Number
- LAT confirmation
- GDC certificate
- UK Blood test report showing Hep B immunisation and titre levels, Hep B, HIV and TB results dated within last 5 years
- Indemnity certificate
- Passport
- Enhanced CRB (within the last 12-36 months)
- Clinical references covering places of employment over past 2 years (minimum 2 referees)
- Degree certificate
- CPR/AED/Medical Emergencies certificate dated within the last 12 months
- PIN number
