Part-time Locum dentist required 3 days per week in a busy, well established INDEPENDENTLY OWNED mixed practice near Leeds, West Yorkshire. Working Tuesday - Thursday 9am to 5.30pm. Practice is fully computerised, digital x-rays, rotary endo, implant kit, air conditioned, modern well equipped surgeries, hygienist, full support team. 10 minutes to Leeds.

Compliance requirements:
- Full NHS Performer Number
- LAT confirmation
- GDC certificate
- UK Blood test report showing Hep B immunisation and titre levels, Hep B, HIV and TB results dated within last 5 years
- Indemnity certificate
- Passport
- Enhanced CRB (within the last 12-36 months)
- Clinical references covering places of employment over past 2 years (minimum 2 referees)
