Locum required for immediate start working 3-5 days (flexible Monday - Friday) on an ongoing basis until April 2019 (Locum to Permanent opportunity 7000 UDAs at £11) in a busy, well established, mixed practice near Lincoln, Lincolnshire. Existing, well maintained list. Fully computerised, digital x-rays, full support team.

Compliance requirements:
- Full NHS Performer Number
- LAT confirmation
- GDC certificate
- UK Blood test report showing Hep B immunisation and titre levels, Hep B, HIV and TB results dated within last 5 years
- Indemnity certificate
- Passport
- Enhanced CRB (within the last 12-36 months)
- Clinical references covering places of employment over past 2 years (minimum 2 referees)
- Degree certificate
- CPR/AED/Medical Emergencies certificate dated within the last 12 months
- PIN number
