Full or part-time Locum required for 3-5 days per week in a very busy, mixed, well established, INDEPENDENTLY OWNED practice near Melling, Merseyside. Practice hours are Monday - Thursday 8:30-5:30 and 8:30-2pm on Friday. Fully computerised, digital X-rays, full support team. 30 minutes from Liverpool and Warrington, 50 minutes to Manchester.

Compliance requirements:
- Full NHS Performer Number
- LAT confirmation
- GDC certificate
- UK Blood test report showing Hep B immunisation and titre levels, Hep B, HIV and TB results dated within last 5 years
- Indemnity certificate
- Passport
- Enhanced CRB (within the last 12-36 months)
- Clinical references covering places of employment over past 2 years (minimum 2 referees)
- Degree certificate
- CPR/AED/Medical Emergencies certificate dated within the last 12 months
